Integrated Services

 

This strategy aims to improve the integration and cohesion of services delivered to disengaged young people.  It recognises that, at present, young people can access multiple services and programs without a coordinating mechanism to ensure services are targeted and complementary.

Brimbank/Melton LLEN is working with key partners such as Youth Connections and YouthNow as well as service provider networks to identify key areas to address and how to address them.

In 2010-2011 the following activities were undertaken with Brimbank/Melton LLEN partners towards this strategic goal:

• Youth Services Forum
• Youth Services Directory (see Youth Services Directory page)
• Youth Connections Triage
• Common referral and information sharing process
• Student engagement and transition teams
